Applicants for the residency program must be eligible for licensure in the State of Minnesota.

Interested applicants should provide the following by the third Friday of January:

• Letter of interest
• Residency application form [print form]
• Curriculum vitae
• Three letters of recommendation from college faculty or employers, using the "Request for Recommendation" form [print form]
